| Q. Which papers/documents/fees, do I take with me? |

- Application on plain paper.
- Affidavit to the following effects:- Details of deceased person i.e.
name, age, address. Details of surviving members of deceased, their
ages and relationship with the deceased. Purpose for which the

- Death Certificate.
- Photocopy of ration card/any other proof of residence in
Delhi.
- Copy of letter/reference of the authority before whom the surviving
members certificate is required (if any).

| Q. What will be the criteria used while deciding
my case? |

| After receiving the application, verification of the facts
is carried out through the Tehsildar. On the basis of the report of
Tehsildar & documents submitted by the applicant, the certificate is
issued to the applicant. |
